Privacy Policy

Sendagaya International Clinic (hereinafter referred to as the “Clinic”) hereby establishes this Privacy Policy (hereinafter referred to as the “Policy”) with respect to the handling of personal information of users (hereinafter referred to as “Users”).

Article 1 (Personal Information)
The term “Personal Information” shall mean “personal information” as defined under the Act on the Protection of Personal Information of Japan, and shall refer to information relating to a living individual which enables identification of a specific individual by name, date of birth, address, telephone number, contact details, or other descriptions contained therein, as well as data such as appearance, fingerprints, or voiceprints, and insurer numbers on health insurance cards, which by themselves can enable identification of a specific individual (personally identifiable information).
Article 2 (Method of Collecting Personal Information)
The Clinic may request Users to provide personal information such as name, date of birth, address, telephone number, and email address when making inquiries or applying for employment. Furthermore, the Clinic may obtain transaction records and settlement information, including personal information of Users, from its business partners (including information providers, advertisers, advertisement distributors, etc.; hereinafter referred to as “Partners”).
Article 3 (Purpose of Collecting and Using Personal Information)
The purposes for which the Clinic collects and utilizes personal information shall be as follows:
  • 1.
    For the proper operation and management of the Clinic;
  • 2.
    For responding to inquiries from Users (including identity verification);
  • 3.
    For contacting Users as necessary, including the provision of important notices; and
  • 4.
    For purposes incidental or related to the foregoing.
Article 4 (Amendment of Purpose of Use)
The Clinic may amend the purpose of use of personal information only where it is reasonably recognized to have a relevant connection with the purpose prior to the amendment. In the event of an amendment, the Clinic shall notify Users of the amended purpose by the method prescribed by the Clinic, or publicly announce the same on its website.
Article 5 (Provision of Personal Information to Third Parties)
The Clinic shall not provide personal information to third parties without the prior consent of the User, except in the following cases, or where such provision is permitted under the Act on the Protection of Personal Information and other applicable laws and regulations:
  • 1. Where it is necessary for the protection of the life, body, or property of an individual, and it is difficult to obtain the consent of the User;
  • 2. Where it is particularly necessary for the improvement of public health or the sound growth of children, and it is difficult to obtain the consent of the User;
  • 3. Where it is necessary to cooperate with a state organ, a local government, or a person entrusted thereby in executing affairs prescribed by laws and regulations, and obtaining the consent of the User is likely to impede the execution of such affairs;
  • 4.
    Where the following matters have been publicly announced in advance and the Clinic has submitted notification to the Personal Information Protection Commission:
    • That the purpose of use includes provision to third parties;
    • The items of data to be provided to third parties;
    • The means or method of provision to third parties;
    • That the provision of personal information to third parties will be discontinued upon request of the User;
    • The method by which requests from the User will be accepted.
      Notwithstanding the foregoing, the following cases shall not constitute provision to third parties:
    • Where the Clinic entrusts all or part of the handling of personal information within the scope necessary to achieve the purpose of use;
    • Where personal information is provided as a result of succession of business due to a merger or other reason;
    • Where personal information is used jointly with a specific party, and the items of jointly used personal information, the scope of joint users, the purpose of use, and the name of the party responsible for the management of the personal information are notified to the User in advance or placed in a readily accessible condition for the User.
Article 6 (Disclosure of Personal Information)
Upon request from the User, the Clinic shall disclose the relevant personal information without delay. However, the Clinic may decline to disclose all or part of the information where disclosure falls under any of the following, and if a decision is made not to disclose, the Clinic shall notify the User without delay. A handling fee may be charged in connection with such disclosure.
  • 1. Where there is a risk of harm to the life, body, property, or other rights and interests of the User or a third party;
  • 2. Where there is a risk of significant hindrance to the proper conduct of the Clinic’s business operations;
  • 3. Where disclosure would result in a violation of applicable laws or regulations.
Notwithstanding the foregoing, information other than personal information, such as historical information or characteristic information, shall not, in principle, be disclosed.
Article 7 (Correction and Deletion of Personal Information)
In the event that personal information retained by the Clinic is incorrect, the User may, in accordance with procedures prescribed by the Clinic, request correction, addition, or deletion of such personal information (hereinafter referred to as “Correction, etc.”).

If the Clinic determines that it is necessary to respond to such a request, the Clinic shall make the Correction, etc. without delay.
Where the Clinic has made a Correction, etc., or has decided not to make a Correction, etc., the Clinic shall notify the User without delay.
Article 8 (Suspension of Use of Personal Information, etc.)
Where the User requests suspension of use or deletion of personal information (hereinafter referred to as “Suspension of Use, etc.”) on the grounds that such information is being handled beyond the scope of the stated purpose of use, or that such information has been obtained through improper means, the Clinic shall conduct the necessary investigation without delay.

If, based on the results of such investigation, the Clinic determines that it is necessary to respond to the request, the Clinic shall implement the Suspension of Use, etc. without delay.
Where the Clinic has implemented a Suspension of Use, etc., or has decided not to implement a Suspension of Use, etc., the Clinic shall notify the User without delay.
Notwithstanding the foregoing, in cases where the Suspension of Use, etc. would involve significant expense or is otherwise difficult to implement, and alternative measures necessary to protect the rights and interests of the User can be taken, the Clinic shall take such alternative measures.
Article 9 (Amendment of the Privacy Policy)
Except as otherwise provided by law or in this Policy, the Clinic may amend the contents of this Policy without notice to the Users. Unless otherwise specified by the Clinic, the amended Policy shall take effect from the time it is posted on the Clinic’s website.
